Southwest Hockey online registration began on September 1. The online registration may be accessed from the home page of this website (scroll down on the home to the bottom) and is still open for Mites, U8, U6 and Mighty Mites. (Squirt, U10, Peewee, U12, Bantam and U14 levels have had tryouts already).
Southwest Hockey Fees are as follows:
M-Mites:/U6: $225
Mites/U8: $425
Squirts/U10: $825
Peewee/U12: $1150
Bantam/U14: $1150
There are four documents that must be completed by ALL registrants (except the Mighty Mites who do not need to complete the Volunteer Agreement)
Players will not be allowed on the ice until all forms and minimum payments have been received.
1) Registration form (complete online or contact Emily Winter at 612-929-3166 if you are unable to do so and we will get a hard copy to you.)
2) Waiver of Liability, Release, Assumption of Risk & Indemnity Agreement (you will complete at the USA Hockey website as part of your online registration)
3) Consent to Treat/Medical History form (print below)
4) SWHA Volunteer Agreement and check (print below)
In addition, we will need a copy of players’ birth certificate(s), issued by a city, county, state or federal agency, if one is not already on file.
Mail your forms and volunteer check to:
SWHA-Registration, P.O. Box 24825, Minneapolis, MN 55424. Check should be made payable to: “Southwest Hockey” or "SWHA."
