We are pleased to inform you that Edina Orthodontic Associates has offered to provide custom Proform/Essix Mouthguards at no charge for the players in the Storm Youth Hockey Program.  

The mouthguard impressions will be taken on Saturday October 4, 2008 at 3939 West 50th Street, Suite 207 in the 50th and France Building. (Next to Lunds)  No appointments are necessary, but please make an effort to come at your appointed time:

Mites 8:00 -   9:30                                                                                
Squirts 9:00 - 10:30     
PeeWees 10:00 - 11:00
Bantams 11:00 - 12:00    
Junior Gold/U16 11:00 - 12:00       

EDINA ORTHODONTICS will have your custom mouthguard ready one week later! They will be given to one SLP/SW Hockey representative to distribute to the appropriate coaches. If an additional mouthguard is desired or you need a replacement during the season, EDINA ORTHODONTICS will gladly fit you for a new one. There will be a charge of $50.00 for this additional mouthguard.  

The MSHL and MAHA have again made the wearing of mouthguards mandatory for those participating in hockey.  The American Dental Association has reported that more that 200,000 injuries are prevented each year by mouthguards.  Although mouthguards can greatly reduce sports injuries,
Dr.’s Liljemark, Holmberg, Vayda and Bowlby cannot be held responsible for any damage to a player’s teeth while wearing these mouthguards.  If you need further information, please call (952) 926-2335.

ATTENTION: If you have orthodontic appliances it is best to use a loose fitting mouthguard, therefore, we will be unable to fit you for a custom mouthguard.  You can purchase a loose filling mouthguard at most sporting goods stores.
